No experience required. Training provided online.
Must currently drive for Uber or Lyft, have a good working vehicle, active drivers license, valid vehicle registration and auto insurance.
Community Health Outreach Workers Assist individuals and communities to adopt healthy behaviors, conduct outreach for medical personnel or health organizations to implement programs in the community that promote, maintain, and improve individual and community health.
You may provide information on available resources, provide social support and informal counseling, advocate for individuals and community health needs. You may collect data to help identify community health needs. You may maintain updated client records with plans, notes, appropriate forms, or related information. You may advise clients or community groups on issues related to improving general health, such as diet or exercise, as well as, identify or contact members of high-risk or otherwise targeted groups, such as members of minority populations, low-income populations, or pregnant women. You may contact clients in person, by phone, by e-mail or in writing to ensure they have completed required or recommended actions.

You will distribute flyers, brochures, or other informational or educational documents to inform members of a targeted community.

You will visit individuals in their homes or in the community to provide support or information.

Transporting patients to and from appointments and Conducting field verification of patient residence locations to facilitate completion of surveys or gathering of information

The positions starts AS NEEDED.

Compensation?
​
We compensate up to $5.00 per completed Patient Survey with visit GPS and descriptive note. We expect a minimum of 20-30 Surveys per day. Additional compensation is provided in the form of Transport requests to and from appointments through your livery service. We offer direct deposit on a bi-weekly basis.

Job Description?
Mobile Field Reports will help clients:
​

Understand the benefits of preventative health care and keeping regular appointments with their providers.

Accurately relay and enter needed client information into software, websites and other information collection sources as require.

Responsibilities?

Educate the member on preventative health care

Comply with all on-boarding, annual and other mandatory training as assigned.

No medical license required, but healthcare experience preferred.
